Nurse Practitioner –
The Center for
Elders Independence is a non profit
organization serving frail elders in the East Bay.
Our innovative, all-inclusive, long-term health care program (PACE Program)
provides the frail elderly with all the care they need, allowing them to
maximize their health & independence and remain close to their family
members and within the community. We currently have an excellent opportunity
for a Nurse Practitioner.
The Position:
Under the supervision of CEI’s Site Physician,
the Nurse Practitioner is responsible for providing care to CEI’s participants in the primary care clinic in
accordance with CEI policies and procedures and Nurse Practitioner
protocols.
DUTIES:
(Primary duties and
responsibilities – may not include all of the duties assigned)
1.
Provides urgent care for acute medical conditions.
2.
Provides follow-up care for chronic medical
conditions.
3.
Performs periodic medical evaluations.
4.
Participates in IDT development of comprehensive
care plans.
5.
Consults appropriately with site physician.
6.
Communicates appropriately with other disciplines
as needed.
7.
Communicates appropriately with outside
consultants, nursing homes, board & care facilities, and family
caregivers.
8.
Documents care provided according to policies and
procedures in the participant medical records in a timely fashion.
9.
Follows all CEI Policies and Procedures, OSHA
safety guidelines.
10.
Maintains the confidentiality of all CEI
procedures, results, and information about participants, clients or
families regardless of setting.
11.
Participates in departmental and agency meetings as
directed.
12.
Completes all required paperwork.
13.
Participates in the agency quality
assurance/performance improvement program (QAPI) as assigned.
14.
Participates in all required departmental or agency
training programs.
15.
Any other duties as assigned.
QUALIFICATIONS:
1.
Masters Prepared Nurse Practitioner—Family, Adult,
or Geriatric certificate and licensed as a Registered Nurse in California. Must be certified by the BRN.
2.
DEA Certificate and Furnishing License required
3.
Experience and interest in working in geriatrics.
4.
A current certification in CPR and First Aid.
5.
Demonstrated ability to work effectively as a part
of an interdisciplinary team.
