Medical Records Technician

The Center for Elders Independence is a non profit organization serving frail elders in the East Bay. Our innovative, all-inclusive, long-term health care program (PACE Program) provides the frail elderly with all the care they need, allowing them to maximize their health & independence and remain close to their family members and within the community. We currently have an excellent opportunity for a Medical Records Technician at our new Eastmont II site!

The Position: Under the technical direction of the Medical Records Coordinator, and the administrative supervision of the Site Manager, the Medical Records Technician (MRT) is responsible for the upkeep and maintenance of the medical records at  the assigned site and provides as needed coverage at other sites during absences, vacations, etc.  The MRT sends for medical information and prepares medical records for new participants, assembles care plans, maintains active, closed and purged records, prepares records for quality activities, conducts quality audits of medical records and identifies documentation deficiencies.  The MRT provides assistance to program staff with medical records, forms and databases related to participant records in a positive and helpful manner.

Duties and Responsibilities: 

1.                  Enters data in to CEI database(s) for the assigned site.

 

2.                  Updates changes to participants medications in PACECare and on-call reports.

 

3.                  Requests medical records from other facilities or participants and perspective participants.

 

4.                  Responds to medical records from participants or family, including getting approval for the release prior to releasing the records.

 

5.                  Responds to questions and requests from the IDT regarding forms, medical records and other pertinent issues.

 

6.                  Conducts quality audits of medical records. Identifies and reports documentation deficiencies to team members and follows up to ensure deficiencies are corrected.

 

7.                  Acts as a liaison to IDT regarding questions about documentation and developing new medical records’ policies.

 

8.                  Provides recommendations to correct deficiencies and to ensure CEI documentation and medical records meet applicable community standards, regulations, and other customer requirements.

 

9.                  Keeps face sheet information current in participants chart and provide copies for the Administrative Assistant for their assigned site.

 

10.              Maintains all medical records including filing documents, thinning charts and purging inactive records according to policies and procedures..

 

11.              Prepares records for specialist visits to ADHC, I&A meetings, Quality Management studies and meetings, surveys by outside agencies and other program needs as required.

 

Qualifications:

 

1.      Three (3) years previous experience in medical and/or administrative capacity with at least two (2) years recent experience as medical clerk/ward clerk in hospital or physician office.  Other relevant health related background will be considered.

 

2.      Knowledge of medical terminology including the ability to use correctly and spell a wide variety of medical terms.  Completion of a medical terminology course preferred.
