Activity Director
The
Center for Elders Independence is an established nonprofit organization
licensed by the California Department of Health Services. Our innovative,
all-inclusive, long-term health care program (PACE Program) provides the
impaired & frail elderly with all the care they need, allowing them to
maximize their health & independence and remain close to their family
members and within the community. We currently have an excellent
opportunity for an Activity Director for our new Eastmont II site!
The Position: Under the supervision of the site
manager, the Activity Director is responsible for developing and carrying
out the therapeutic recreational activity programs for CEI participants.
The Activity Director works with the Activity Coordinator to ensure that
the programming is consistent and collaborative agency-wide. The Activity
Director will develop a recreation program that assists participants in
resuming self-care and normal activities and/or preventing further decline.
In addition, the Activity Director supervises the recreation staff and
volunteers.
DUTIES:
(Primary
duties and responsibilities – may not include all of the duties assigned)
1. Conducts
assessments of the participant’s needs and interests for the development of
the individual plan of care by the interdisciplinary team.
2. Conducts
accurate and timely quarterly reviews of the participant’s plan of care.
3.
Provides orientation, on-going training and supervision to activity
aides and volunteers.
4. Develops
a monthly schedule of activities that is diverse, reflects current trends
in activity program and addresses the functional and cultural needs of the
site participants.
5. Posts
the schedule of planned activities in a central location that is accessible
to participants, families, guardians or authorized representatives.
6. Solicits
input from participants in the planning of the recreational activity
program.
7. Recruits
and oversees volunteers to enhance the group and individual activities.
8. Coordinates
with the transportation and nutrition departments and to provide meals and
safe transportation to and from activities in the community (outings).
9. Meets
independent and group needs of all participants and ensures that
participants have time to engage in activities of their own choosing.
10. Facilitates variety of
individual and group entertainment or arts & craft activities, special
events etc to meet the needs of participants at different levels of
functioning.
11. Documents and reports to
the interdisciplinary team any observed changes in participant’s behavior,
including ambulation, eating habits, or level and type of social
interaction
12. Leads the planning and
implementation of new activity groups, including utilizing community
organizations and various other resources such as involving community
groups and schools to enhance the program and add diversity.
13. Prepares periodic
reports and keeps appropriate records.
14. Ensures that the day
room decorations are appropriate, stimulating and enhance the appeal of the
therapeutic environment.
QUALIFICATIONS:
Have
two years of experience in a social or recreational program providing and
coordinating services for a frail or elderly population within the past
five years, one year of which was full-time employment in a patient
activities program in health care setting.
